Community Inclusion Specialist (Per Diem Float)

As a Community Inclusion Specialist, you'll provide direct support, job coaching, life skills training, and community engagement opportunities that help individuals thrive at work and in their daily lives.
What You'll Do:
- Support individuals in community, vocational, and day program settings
- Teach daily living, employment, and social skills
- Assist individuals in achieving personal goals and increasing independence
- Provide transportation as needed
- Support community participation and recreational activities
- Assist with personal care when required
- Complete documentation and program records
- Promote safety and follow individual support plans
- Attend required trainings and meetings
Qualifications:
- High School Diploma or equivalent required
- Associate's Degree preferred
- At least 1 year of related experience preferred
- Must be 21 years or older
- Valid driver's license and clean driving record
- Strong communication and problem-solving skills
- Ability to work independently and as part of a team
- Basic computer skills (Microsoft Office and email)
Position Details:
- Per diem/float position
- Minimum commitment of 8 hours per month
- Program hours: Monday–Friday, 8:00 AM–2:00 PM
- Training and certifications provided as required
