Position Overview
Reign-Bow Brain Treatment Center is seeking a knowledgeable and collaborative Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) to conduct consultations and follow-up appointments for patients participating in or considering transcranial magnetic stimulation (TMS) services.
The PMHNP will educate patients and caregivers, evaluate symptoms and treatment history, monitor progress, identify safety concerns and provide clinically appropriate recommendations to the treatment team.
This is a hybrid position. Most appointments may be conducted through secure telemedicine; however, the PMHNP must work at the Lombard clinic one scheduled day each month.
This position is focused on consultations, follow-ups and clinical recommendations. The PMHNP is not being hired for insurance credentialing or to provide an NPI for Reign-Bow’s medical billing.
Primary Responsibilities
- Conduct initial clinical consultations for patients considering TMS.
- Complete scheduled progress and follow-up appointments during treatment.
- Conduct post-treatment and maintenance-treatment consultations.
- Review psychiatric, medical, developmental, medication and treatment histories.
- Evaluate current symptoms, functional concerns and treatment goals.
- Perform mental-status examinations and appropriate risk assessments.
- Review previous medication and psychotherapy treatment history.
- Identify potential TMS contraindications, precautions and safety concerns.
- Review available clinical information and patient-reported outcome measures.
- Educate patients and caregivers about the general TMS process, expected treatment course, potential benefits, limitations and possible adverse effects.
- Explain the importance of attending treatment consistently and reporting symptom changes.
- Monitor treatment tolerance, symptom changes, functioning and patient or caregiver concerns.
- Assess reported headaches, discomfort, mood changes, sleep changes, behavioral changes or other possible adverse effects.
- Identify situations requiring treatment to be paused or escalated for physician review.
- Provide clinical recommendations to the Medical Director and treatment team.
- Recommend additional psychiatric, psychological, medical or therapeutic evaluation when appropriate.
- Help determine when a patient should be referred to a psychiatrist, primary-care physician, therapist, emergency department or another specialist.
- Document consultations, follow-ups, findings and recommendations accurately and promptly.
- Participate in selected case-review meetings.
- Remain reasonably available for questions regarding patients personally evaluated by the PMHNP.
TMS Knowledge and Clinical Expectations
The successful candidate should understand or be willing to develop competency in:
- General TMS indications and contraindications.
- TMS safety screening.
- Treatment-resistant depression.
- Medication and treatment-history review.
- Seizure-risk considerations.
- Metal and implanted-device screening.
- Common TMS side effects and adverse-event escalation.
- Symptom and functional progress monitoring.
- Standardized behavioral-health outcome measures.
- Maintenance-treatment considerations.
- The clinical distinction between FDA-cleared, off-label and potentially noncovered uses of TMS.
- The importance of medical-director review and payer-specific requirements.
- Appropriate clinical boundaries when treating children, adolescents and neurodivergent patients.
The PMHNP may provide clinical observations and recommendations but must work within the limits of Illinois law, national certification, clinic policy and the authority assigned by the Medical Director. Final TMS ordering, protocol approval and physician-only decisions will remain with the appropriately qualified Medical Director when required.
Patient Population
The PMHNP may work with:
- Adults and adolescents, depending on the provider’s certification and experience.
- Patients with depression, anxiety, trauma-related symptoms, attention concerns or emotional dysregulation.
- Neurodivergent patients, including individuals with autism.
- Parents and caregivers participating in consultations for minor or dependent patients.
- Patients returning for maintenance treatment.
Experience with autism and communication differences is strongly preferred. The PMHNP must communicate clearly and avoid promising guaranteed outcomes.
Collaborative Responsibilities
- Collaborate with Reign-Bow’s founder, Medical Director and treatment team.
- Offer clinical insight regarding patient progress and treatment concerns.
- Participate in scheduled remote case reviews.
- Recommend improvements to consultation and follow-up workflows.
- Help develop appropriate clinical screening and escalation procedures.
- Advise the team when symptoms or risks require additional evaluation.
- Maintain independent professional judgment regarding every patient personally evaluated.
Required Qualifications
- Master’s or doctoral degree in nursing from an accredited program.
- Active, unrestricted Illinois RN and APRN licenses.
- Current national PMHNP-BC certification.
- Certification appropriate to the ages of patients accepted by the provider.
- Illinois Full Practice Authority or a legally compliant collaborative arrangement when required.
- Current professional liability insurance.
- Ability to provide telemedicine services to patients physically located in Illinois.
- Experience performing psychiatric consultations and follow-ups.
- Understanding of psychiatric medications and behavioral-health treatment planning.
- Strong risk-assessment, communication and documentation skills.
- Ability to attend one required in-person clinic day each month in Lombard.
- Successful completion of license, background, exclusions and sanctions verification.
Preferred Qualifications
- Direct TMS experience or formal TMS education.
- Experience evaluating patients before, during or after a TMS treatment course.
- Experience with treatment-resistant depression.
- Experience working with children or adolescents.
- Experience with autism, ADHD, anxiety, depression, trauma or emotional dysregulation.
- Experience communicating with parents and caregivers.
- Telepsychiatry experience.
- Experience working within a collaborative outpatient practice.
Candidates with strong psychiatric experience who are willing to complete approved TMS education may also be considered.
Schedule and Work Arrangement
- Part-time hybrid independent-contractor position.
- Approximately 0–8 hours per week initially.
- Most appointments will be conducted remotely through secure telemedicine.
- One required in-person clinic day per month at Reign-Bow Brain Treatment Center in Lombard.
- The monthly on-site date will be scheduled in advance.
- The in-person day may include consultations, follow-ups, case reviews and team collaboration.
- Some afternoon or early-evening availability is preferred.
- Additional hours may become available as patient volume grows.
- Candidates must live within reasonable traveling distance of Lombard.
Compensation
- $63–$70 per hour.
- Target rate of $70 per hour, based on relevant experience.
- Paid responsibilities include scheduled consultations, follow-ups, required documentation and approved case-reviews.
- Administrative time outside the established role requires prior approval.
- This position does not require the PMHNP to participate in payer credentialing or provide their NPI for Reign-Bow’s insurance billing.
