Full Job Description

Physical Therapist – Home Health (Contract)
Location: Delaware 19904
Contract Length: 13 Weeks
Pay: $2,159 – $2,421/week

Schedule:

Day Shift (8-hour shifts)
Monday–Friday
40 hours guaranteed

Position Summary & Responsibilities (Concise)

Provide home health PT evaluations and treatments
Develop and implement individualized care plans
Document visits in HCHB (Homecare Homebase)
Monitor patient progress and adjust treatment plans
Educate patients/caregivers and coordinate care

Requirements

Education & Experience:

Graduate of accredited Physical Therapy program: Required
Home Health experience: Required
HCHB (Homecare Homebase) experience: Required

Licensure & Certifications:

Active Physical Therapist License (state of practice): Required
BLS/CPR certification: Preferred

Additional Notes

Strong documentation and time management skills required
Must be comfortable working independently in home settings
