The Purchase Area Development District is seeking an individual to work in various Aging programs. The Aging Case Manager is responsible for Title III C meal assessments, Title III D HomeMeds assessments, being a backup to the Long-Term Care Ombudsman, SHIP, and Veteran’s Directed Care programs.
The ideal candidate will have a bachelor’s degree in health or human services with one year of related experience.
Responsibilities include assessing consumers for eligibility, identifying consumer needs and appropriate referral to services other than those contracted through Title III C and Title III D, providing necessary follow-ups with clients, working cooperatively with various agencies, adhering to all state regulations guidelines and policy and procedure requirements of the programs, and completing all necessary paperwork and electronic data entry required, etc.
Skills needed include excellent verbal and written communication, self-direction and excellent organizational skills, efficiency and attention to detail, and the ability to relate appropriately to consumers and other professionals. Knowledge of or ability to learn and effectively utilize program related computer software required. The PADD offers an attractive compensation program and is an EOE/AA/ADA employer.
Pay: $32,000.00 - $34,000.00 per year
