EXAMPLE OF DUTIES
Under general supervision, responsible for day-to-day project implementation, including intake and eligibility confirmation, structured assessment, coordination of POSC development, service planning, and ongoing monitoring. Serves as the primary liaison with DFCS, healthcare providers, and service partners and ensures timely contact, POSC development, service linkage, and documentation in accordance with DFCS requirements. This position is a Grant Funded position.
