What You'll Be DoingLead Patient Care
- Complete comprehensive nursing assessments and individualized care plans
- Perform OASIS visits and ongoing case management
- Coordinate interdisciplinary care and patient transitions
- Educate patients and families with compassion and confidence
- Serve as an advocate throughout the continuum of care
Use Your Clinical Expertise
- Manage complex medical conditions
- Identify changes in condition and intervene proactively
- Collaborate closely with physicians and healthcare teams
- Maintain accurate documentation and regulatory compliance
- Help reduce unnecessary hospitalizations and emergency room visits

What You BringRequired
- Active Minnesota RN license
- Three or more years of nursing experience
- Strong assessment and communication skills
- Reliable transportation and valid driver's license
- Passion for independent, relationship-based nursing
Preferred
- Home health experience
- OASIS experience
- Previous case management experience
