ABOUT SFHDC
The San Francisco Housing Development Corporation (SFHDC) enhances housing stability and financial self-reliance for low-income residents, particularly in communities of color. African American community leaders founded SFHDC in 1988 to combat widespread displacement in the Fillmore and Bayview neighborhoods caused by urban renewal. Today, we empower San Francisco’s Black residents to remain in San Francisco or return if displaced. Annually, our integrated programs serve 5,000 people and have developed or preserved over 1,700 permanently affordable homes. Additionally, our Financial Empowerment Program provides over 1,000 clients with credit repair, rental-readiness, and financial capability training, which anchors our homelessness prevention efforts.
PROGRAM DESCRIPTION
Ready, Set, Grow: Homelessness Prevention & Housing Stability Through Financial Health
Ready, Set, Grow is SFHDC’s homelessness prevention and housing retention initiative funded by Crankstart. The program integrates financial coaching, matched savings (IDAs), rental stabilization assistance, and wraparound case management to help at-risk households remain stably housed while building long-term financial security.
This program is a collaborative effort between SFHDC’s Financial Empowerment Center (FEC) and Resident Services. FEC provides financial coaching and education, while Resident Services leads resident engagement, case management, and housing stabilization support within SFHDC-managed RAD and non-senior housing sites.
POSITION SUMMARY
The Case Manager delivers holistic, client-centered housing stabilization and homelessness prevention services to vulnerable residents. As eviction moratoriums end and ERAP funding becomes finite, this role provides critical, urgent intervention to mitigate severe housing stability risks. Collaborating with the Financial Empowerment Center (FEC) and Resident Services, the Case Manager anchors the client experience by creating personalized stabilization plans and connecting residents to essential wraparound services, such as mental health support, employment assistance, and public benefits. The Case Manager identifies risks early, provides consistent monthly support, and coordinates pathways to financial resilience through Individual Development Accounts (IDAs) and SF ERAP assistance. By tracking housing retention and financial outcomes, the Case Manager ensures the program meets key deliverables and demonstrates measurable community impact. Crucially, while launched as a specialized initiative, this position is designed for long-term viability and will be sustained through property management service fees once a measurable impact is demonstrated.
CORE RESPONSIBILITIES
READY – Assessment & Risk Identification
- Conduct intakes and needs assessments to evaluate financial health, housing risks (e.g., eviction, past-due rent), and stability barriers.
- Prioritize vulnerable populations, including families, individuals with disabilities, domestic violence survivors, and those with former justice involvement or recent homelessness.
- Administer self-assessments to gauge financial stress and personal stability goals.
- Identify underlying barriers (e.g., mental health, substance use) and collaborate with Property Managers to assign housing risk levels
SET – Stabilization & Support
- Develop customized stabilization plans featuring specific action steps, timelines, and wraparound service referrals.
- Lead monthly check-ins to monitor progress, update plans, and resolve emerging crises.
- Facilitate referrals and enrollments for SF ERAP, JobsNOW!, public benefits (e.g., CalWORKs, MediCal), on-site mental health, Casa de Las Madres, and Westside Community Health.
- Partner with FEC staff to enroll clients in Patelco Credit Union Individual Development Accounts (IDAs) and financial literacy workshops
GROW – Savings & Long-Term Housing Stability
- Monitor rent payments to achieve program milestones, including transitioning 85% of households to automatic payments.
- Guide participants in leveraging 2:1 matched savings (100/month) to build emergency funds or clear arrears.
- Track housing retention outcomes to meet the goal of preventing evictions for 95% of participants.
- Collaborate with the Policy and Data Manager to measure financial outcomes, data accuracy, and behavioral shifts.
SKILLS & QUALIFICATIONS
Required
- Possess 2-4 years of experience in case management, housing stabilization, homelessness prevention, or behavioral health.
- Deliver culturally attuned support to low-income communities of color.
- Engage vulnerable populations, including families, individuals with disabilities, domestic violence survivors, and those with justice involvement or recent homelessness.
- Navigate systemic barriers alongside clients, addressing housing instability, financial hardship, substance use, and mental health.
- Maintain detailed case notes for specialized funder tracking.
- Utilize Microsoft Office and Google Workspace to track participant demographics and risk data.
- Bridge efforts between internal teams (e.g., Financial Empowerment Center) and external partners.
Preferred
- Bachelor’s degree or higher in Social Work, Psychology, Counseling, or a related field.
- Navigate emergency rental subsidies (specifically SF ERAP) and facilitate public benefits enrollments (e.g., CalWORKs, General Assistance, CalFresh, MediCal, SSI).
- Understand financial capability initiatives (e.g., IDAs) and basic savings milestones.
- Administer self-assessments to track qualitative shifts in financial habits, attitudes, and emotional wellness.
- Collaborate with local resources (e.g., JobsNOW!, Casa de Las Madres, Westside Community Health), leveraging deep knowledge of San Francisco’s social services.
WORK ENVIRONMENT & SPECIAL REQUIREMENTS
- Location & Travel: Travel regularly between San Francisco SFHDC sites (specifically Bayview and Western Addition). Maintain a valid California driver’s license and reliable transportation.
- Work Setup: Maintain reliable internet for remote or hybrid work.
- Schedule: Work primarily on weekdays, accommodating occasional evenings or weekends to meet resident needs and facilitate workshops and events.
COMPENSATION & BENEFITS
- Salary Range: $60,000 – $70,000 annually, commensurate with experience.
- Benefits: SFHDC offers a competitive and comprehensive benefits package, including:
