Job Overview:
The Direct Supports Professional- DSP reports to the Program Manager and is responsible for assisting, training, and supporting our clients. The DSP is responsible to provide excellent services according to the client’s person-centered plan. The objective of the DSP is to assist the clients in their daily living activities and-or employment goals to positively impact each client’s growth.
The DSP provides direct services to our clients- no matter the department. The DSP is also considered a caregiver in our residential-respite programming, and a job coach in our employment programming. The Center has incorporated each one of the ‘titles’ into one job description due to the fact the employee will cover multiple departments; however, the goal does not change—assist the client to achieve their personal goals to positively impact the client’s growth and independence—No matter the department.

· Facilitate effective and meaningful activities (monthly calendars) tailored to business and client needs.
· Facilitate training sessions with the clients on various topics, including safety while in the community, recipient rights, compliance-reporting, and personal development.
· Follow each client’s person-centered plan to aid in achieving their goals.
· Meet with clients on a regular basis to ensure active participation, communication and client engagement.
· Stay informed on trends, best practices, regulatory changes by participating in monthly departmental meetings.
· Observe and report any patterns in behavior including any progression or regression and report to the Program Manager, capture success stories.
· Acts as a resource to the clients, including supporting and motivating them when possible.
· Perform other related duties as assigned.
Caregiver:
· Personal Care: Assist with bathing, dressing, grooming, and toileting.
· Mobility: Assist with transferring, walking, and using mobility devices.
· Medication Management: Remind clients to take medications and monitor for side effects or medication administration based on abilities.
· Meal Preparation: Prepare and serve nutritious meals according to dietary needs and preferences.
· Light Housekeeping: Maintain a clean and organized living environment.
· Companionship: Provide emotional support and engage in meaningful activities.
· Transportation: Transport clients to appointments and errands.
· Interact with clients to assist them in gaining insight, defining goals, and planning action to achieve effective personal, social, educational, and vocational development and adjustment.
· Documentation: Maintain accurate records of care provided.
Community Coach- Employment and-or CLS:
· Create monthly calendar plannings with client
· Meet with clients on a weekly basis to provide services according to their person-centered plan.
· Provide transportation to participate in community activities, access resources and build relationships
· Support clients to complete daily tasks as independently as possible
· Complete/maintain client documentation
· Assist clients by assisting with maintaining their employment, which includes on-the-job training.
· Training with safety skills, including evacuation at their current location
Requirements:
· Education: High School Diploma.
· Experience: The Center will train the required skillset; however, the employee will be an active member by engaging in the training courses.
· Physical Requirements
· The Direct Support Professional position will typically work in the community or in the client’s home. There will be occasional work outside and in small, enclosed spaces. Frequent sitting, standing, talking, active listening and repetitive motions.
· Occasional lifting of 50 lbs. Occasional bending, twisting, stooping, squatting, pushing, pulling, crawling, reaching, walking, driving, traveling, kneeling and crouching.
· Caregivers will transfer clients.
