As a Registered Nurse at Cottage Home Care, you will play a critical role in evaluating patient needs, conducting comprehensive assessments, establishing personalized care plans, and supervising direct care staff. You will work independently in community-based settings while relying on robust digital documentation tools to deliver exceptional patient-centered care.
Key Responsibilities
- Conduct comprehensive home healthcare clinical visits, including Head-to-Toe physical assessments and specialized evaluations.
- Complete Uniform Assessment System (UAS-NY) assessments accurately and within required state timeframes.
- Develop, implement, and update individualized Plans of Care (POC) tailored to patient strengths, health risks, and daily living needs.
- Utilize the Health Commerce System (HCS) to review, input, and manage official state health documentation.
- Use HHAeXchange for efficient clinical documentation, visit tracking, and communication with the administrative and coordination teams.
- Perform supervisory visits and competency evaluations for Home Health Aides (HHAs) and Personal Care Aides (PCAs).
- Monitor patient outcomes, recognize changes in physical or cognitive condition, and coordinate care with physicians, case managers, and family members.
Required Qualifications
- License: Active, unencumbered New York State Registered Nurse (RN) license.
- Experience: Minimum of 2 years of active Registered Nursing experience in Home care is required.
- Clinical Skills: Strong proficiency in conducting thorough Head-to-Toe physical assessments and clinical documentation.
- Autonomy: Ability to manage field-based appointments, work independently, and submit clinical documentation efficiently.
- Drivers & Travel: Valid driver’s license, reliable personal vehicle, and auto insurance for local field visits in ZIP code 11520 and surrounding Nassau County areas.
Preferred Qualifications
- Previous home healthcare or community-based nursing experience strongly preferred.
- Hands-on experience with UAS-NY assessments.
- Familiarity with HHAeXchange software and the New York Health Commerce System (HCS) portal.
Compensation & Benefits
- Hourly Pay: $50.00 per hour (~$104,000 annually for full-time schedule)
