Social Worker – Day Program
Organization: Beyond 21
Department: Adult Day Program
Reports To: Day Program Director
Status: Full-Time, Exempt/Non-Exempt (as applicable)
Work Schedule: Monday–Friday, 8:30 a.m.–5:00 p.m. (40 hours/week; 30-minute unpaid lunch)
Additional Hours: Occasional evenings, weekends, and holidays as required.
Position Summary
Beyond 21 is seeking a dedicated and compassionate Social Worker to provide direct support and case management services for adults with intellectual and developmental disabilities (I/DD) enrolled in our Day Program. The Social Worker plays a vital role in promoting participant well-being, independence, community inclusion, and person-centered services through collaboration with participants, families, interdisciplinary team members, and community partners.
Essential Duties and Responsibilities
Provide direct support and actively participate in the implementation of daily program activities designed to promote participant engagement, independence, and personal growth.
Conduct participant assessments and assist with intake, admissions, and orientation processes as assigned by the Day Program Director.
Provide ongoing case management, including monitoring participant progress, coordinating services, advocating for participant needs, and maintaining communication with families, guardians, and external providers.
Develop, maintain, and update participant records in accordance with agency policies and applicable state and federal regulations.
Complete timely, accurate, and comprehensive documentation, including case notes, assessments, incident reports, and service plans.
Participate as an active member of the interdisciplinary team by collaborating with program staff, clinicians, community agencies, and other professionals to ensure coordinated, person-centered care.
Attend and participate in required staff meetings, interdisciplinary meetings, trainings, and professional development activities.
Utilize crisis prevention, de-escalation, and behavior management techniques to ensure participant safety while maintaining dignity and respect.
Support participants in achieving individualized goals and maximizing community integration and quality of life.
Perform other duties as assigned by the Day Program Director.
Qualifications Required
Bachelor's Degree in Social Work (BSW) from an accredited college or university.
Current knowledge of social work principles, ethics, and case management practices.
Excellent interpersonal, communication, organizational, and documentation skills.
Ability to work collaboratively within a multidisciplinary team.
Preferred
Master's Degree in Social Work (MSW).
Experience working with individuals with intellectual and developmental disabilities.
Experience with person-centered planning, behavioral supports, and community-based services.
Knowledge, Skills, and Abilities
Understanding of the unique needs of adults with intellectual and developmental disabilities.
Strong assessment, problem-solving, advocacy, and case management skills.
Ability to establish positive relationships with participants, families, and community partners.
Ability to maintain confidentiality and comply with HIPAA and agency policies.
Proficiency in electronic documentation and Microsoft Office applications.
Ability to remain calm and effective during crisis situations.
Physical Requirements
Ability to stand, walk, bend, and assist participants throughout the program day.
Ability to respond quickly during behavioral or emergency situations.
Ability to lift and/or assist with up to 25 pounds, with reasonable accommodations as applicable.
