Responsibilities
Provide patient-focused physical therapy assessments and treatment plans.
Deliver evidence-based PT interventions to improve functional outcomes.
Document patient progress and update care plans in the electronic health record.
Collaborate with a multidisciplinary team to coordinate care.
Demonstrate accountability for individual targets and performance metrics.
Maintain compliance with clinical guidelines and safety protocols.
Contribute to and track patient care pipelines to forecast outcomes.
Identify opportunities for professional development and skill advancement.
Engage patients and families to promote adherence and education.
Participate in on-site team meetings and quality improvement initiatives.
Support growth targets by identifying potential referrals and community outreach.
Demonstrate curiosity and learning mindset through continuous education.
Experience/Qualifications
PT licensure with recent graduate eligibility and supervision plan.
Completion of entry-level physical therapy assessment and plan development.
Evidence-based PT interventions and outcome-driven treatment delivery.
Adherence to clinical guidelines, safety, and infection control protocols.
Professional development plan with tracked milestones and continuing education.
Documentation of patient progress in electronic health records.
Multidisciplinary collaboration and care coordination.
Participation in care pipelines, forecasting patient outcomes and referrals.
Patient and family engagement to promote adherence and education.
On-site participation in team meetings and quality improvement initiatives.
Active participation in community outreach and referral growth strategies.
