We are seeking a compassionate, dependable, and experienced Licensed Vocational Nurse (LVN) to join our home health team. The LVN will provide nursing care directly in patients' homes according to an individualized Plan of Care developed by the Registered Nurse (RN).

Our patient population is approximately 95% Spanish-speaking, so the ability to communicate effectively in both Spanish and English is required.
Schedule & Flexibility
Our LVNs have the flexibility to create their own schedules based on patient needs and availability.
Nurses are responsible for coordinating their visits with patients and maintaining consistent visit schedules according to each patient's Plan of Care.
This position offers the independence of working in patients' homes while remaining supported by our clinical team.
Responsibilities
- Provide skilled nursing services to patients in their homes according to the RN-developed Plan of Care.
- Perform routine nursing assessments and monitor patient condition.
- Obtain and document vital signs, including blood pressure, pulse, respirations, temperature, and oxygen saturation as appropriate.
- Provide patient and family education regarding medications, disease processes, treatments, safety, and other healthcare needs.
- Administer medications and treatments as ordered and permitted within the LVN scope of practice.
- Perform wound care and other nursing treatments as indicated in the Plan of Care.
- Monitor patients for changes in condition and promptly report concerns to the supervising RN or appropriate healthcare professional.
- Communicate effectively with patients, family members, caregivers, physicians, RNs, and other members of the healthcare team.
- Follow the patient's individualized Plan of Care and agency policies and procedures.
- Accurately document all visits, assessments, interventions, patient responses, and communications.
- Maintain patient confidentiality and comply with HIPAA and applicable healthcare regulations.
- Promote patient safety, independence, and quality of life in the home environment.
- Collaborate with the RN and other members of the care team to ensure continuity and quality of care.
- Perform other LVN duties within the nurse's scope of practice and as indicated by the patient's Plan of Care.
Qualifications
- Current and unrestricted LVN/LPN license in the state of practice.
- Current CPR/BLS certification.
- Previous home health experience preferred, but not required depending on experience.
- Fluent in Spanish and English – required.
- Strong clinical and communication skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and ability to travel to patients' homes.
- Compassionate, professional, and patient-centered approach to care.
- Ability to accurately complete required documentation and communicate changes in patient condition.
- Must be comfortable working independently in a home setting.

Pay: $30.00 - $35.00 per hour
