Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts and other assistive devices
- Take and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates
- Support medication administration under supervision, ensuring proper documentation in electronic health records (EHR) systems such as Epic or Cerner
- Monitor patients’ conditions closely, report changes promptly to nursing staff, and document observations thoroughly in medical records
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral health support
- Assist with clinical procedures like catheter care, phlebotomy preparation, and aseptic techniques in various settings including hospital intensive care units (ICU), emergency departments, or outpatient clinics
- Support patients with complex needs such as those requiring hospice or palliative care, ensuring comfort and dignity at all times
Skills
- Proven experience working in long-term care facilities, nursing homes, assisted living communities, or hospitals with critical care units such as Level I Trauma Centers or PICUs
- Strong knowledge of medical terminology, physiology, anatomy, and HIPAA regulations to ensure compliant patient care and documentation
- Familiarity with EMR/EHR systems including Epic, Cerner, eClinicalWorks or similar platforms for accurate medical records management
- Ability to perform basic life support (BLS) and demonstrate competency in infection control practices and aseptic techniques
- Experience caring for individuals with developmental disabilities or behavioral health needs; adept at handling dementia and memory care situations with patience and professionalism
- Skilled in using medical equipment such as Hoyer lifts, catheters, IV supplies (DME), and other assistive devices safely and effectively
- Excellent communication skills to collaborate seamlessly with multidisciplinary teams across inpatient, outpatient, home & community care settings
