Position Summary
The Navigation Specialist provides client-centered case management, resource navigation, and care coordination services to support individuals and families in achieving stability, self-sufficiency, and overall well-being. This position conducts comprehensive needs assessments, develops individualized service plans, coordinates referrals, and assists clients in accessing community resources, public benefits, healthcare, housing, employment services, and other supports.
Key Responsibilities
Client Assessment and Service Planning
- Conduct client intake interviews and comprehensive needs assessments.
- Develop individualized service plans utilizing SMART goals.
- Monitor client progress toward short-term and long-term goals.
- Provide ongoing follow-up and support to ensure service engagement and continuity of care.
Case Management and Care Coordination
- Provide rapid case management and crisis stabilization support.
- Coordinate internal referrals to behavioral health, medical, housing, employment, and supportive services.
- Facilitate external referrals to community agencies, government programs, and specialty providers.
- Collaborate with multidisciplinary teams and community partners to ensure coordinated service delivery.
- Maintain communication with referral sources and service providers regarding client progress and service needs.
Resource Navigation and Advocacy
- Assist clients in obtaining vital documents, including identification cards, birth certificates, Social Security cards, and immigration documentation.
- Support applications for public benefits, including Medicaid, SNAP, SSDI/SSI, TANF, LEAP, and other assistance programs.
- Provide advocacy and resource navigation to reduce barriers to services and self-sufficiency.
- Educate clients regarding available community resources and support systems.
Housing, Employment and Community Support
- Assist clients with housing navigation, shelter placement, and permanent housing applications.
- Support employment and vocational resource connections.
- Coordinate transportation to appointments, housing interviews, treatment programs, and community resources.
- Assist with family reunification and reconnection to natural supports when appropriate.
- Support discharge planning and transition planning for clients exiting services
Documentation and Compliance
- Document client interactions, interventions, referrals, service plans, and outcomes in accordance with agency, funding, and regulatory requirements.
- Maintain accurate and timely case records.
- Complete service plan updates and required reports.
- Track client outcomes and program performance measures.
Required Qualifications
- High school diploma or GED required. Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
- Minimum of one (1) year of experience working with individuals experiencing homelessness or other social services field
- Knowledge of community resources, public benefits systems, housing programs, and supportive services.
- Ability to work effectively with individuals experiencing mental health challenges, substance use disorders, trauma, and housing instability.
- Strong organizational, communication, documentation, and interpersonal skills.
- Ability to maintain professional boundaries and confidentiality.
- Proficiency with Microsoft Office applications.
- Valid driver's license and ability to transport clients when required by agency policy.

- Must maintain a drug-free status (including THC) and be willing to submit to pre-employment and random drug and alcohol screenings in accordance with organizational policy and applicable law

Benefits
- Compensation dependent on experience, beginning at $25/hour
- Paid time off plan begins at 2-weeks (10 days) per year.
- Sick time is accrued at a rate of 1 hour per every 30 hours worked

Pay: $25.92 - $31.22 per hour
