We are seeking a dedicated and compassionate Registered Nurse to serve as a Hospice Case Manager. This vital role involves providing comprehensive end-of-life care, supporting patients and their families through difficult times with professionalism and empathy. The ideal candidate will possess extensive clinical experience, including critical care, emergency medicine, or hospital-based nursing, and demonstrate a strong commitment to patient-centered care. As a Hospice Case Manager, you will coordinate services, manage patient assessments, and ensure seamless delivery of hospice care in compliance with healthcare regulations.
Duties
- Conduct thorough patient assessments to determine eligibility for hospice services and develop individualized care plans aligned with patient needs and preferences.
- Manage clinical case management activities, including symptom management, medication administration, and monitoring of patient comfort levels.
- Collaborate effectively with multidisciplinary healthcare professionals such as physicians, social workers, and therapists to coordinate comprehensive hospice care.
- Oversee discharge planning and transition processes from hospital or inpatient settings to home or hospice facilities, ensuring continuity of care.
- Utilize electronic health records (EHR/EHR) systems to document patient information accurately while maintaining compliance with HIPAA regulations.
- Provide education and emotional support to patients and families regarding disease progression, treatment options, and end-of-life decisions.
- Conduct regular patient visits to monitor health status, vital signs, and response to treatment plans while adjusting care strategies as needed.
- Engage in utilization management activities to optimize resource use and ensure appropriate level of care for each patient.
Skills
- Extensive clinical experience in critical care environments such as ICU, Level I Trauma Centers, or Emergency Rooms.
- Proven expertise in hospital medicine, post-acute care, or home health nursing with familiarity in medical-surgical and outpatient settings.
- Strong knowledge of health regulation policies including HIPAA compliance and medical terminology.
- Competence in managing complex cases involving infectious disease care, geriatrics, behavioral health, or pediatrics.
- Skilled in patient assessment, diagnostic evaluation, discharge planning, and case management strategies.
- Proficiency with EMR/EHR systems for documentation and medical records management.
- Excellent communication skills for client interaction and collaboration with healthcare teams.
- Ability to adapt quickly in high-pressure situations while maintaining a focus on patient service excellence.
