Firefly Outpatient is seeking a dedicated and organized Case Manager to join our clinical team. This role plays a critical part in coordinating client admissions, treatment planning, discharge planning, and aftercare services. The Case Manager serves as a key liaison between clients, families, referral sources, and the interdisciplinary treatment team to ensure seamless continuity of care.
The ideal candidate is compassionate, detail-oriented, and experienced in substance use disorder and/or mental health treatment environments, with strong communication and crisis intervention skills.
Primary Responsibilities
- Direct the response to requests for services, including intake calls, assessments, referrals, level-of-care determinations, and follow-up.
- Manage admissions calls (including evenings and weekends as assigned).
- Oversee client admissions and ensure all intake documentation is accurate and compliant with licensing and Joint Commission standards.
- Collaborate with the Clinical Director to evaluate appropriateness of admissions.
- Maintain active awareness of all client admissions, discharges, and transfers.
- Coordinate discharge planning, aftercare services, relapse prevention planning, and outcome follow-up.
- Meet weekly with assigned clients to support treatment planning and progress.
- Facilitate sessions with referral sources and family members as needed.
- Conduct crisis intervention and de-escalation as required.
- Maintain ongoing communication with referral partners and support marketing initiatives as assigned.
- Ensure compliance with HIPAA, ODMHSAS, Joint Commission, and agency standards.
- Maintain accurate, timely documentation in the electronic health record.
- Participate in weekly meetings and performance improvement initiatives.
- Perform other duties as assigned.

Qualifications
- Must be at least 18 years of age
- Bachelor’s degree in behavioral health, business, marketing, administration, or related field (preferred) or equivalent experience
- Registered, certified, or licensed by state-approved certifying organization for behavioral health / addiction
- Minimum of two (2) years of experience in a healthcare or behavioral health setting (preferred)
- Knowledge of psychiatric and chemical dependency treatment principles (preferred)
- CPR/First Aid certification (or ability to obtain upon hire)
- Valid Class “C” Driver’s License (if driving for company) or valid Photo ID
- Successful completion of background check and health screening
- Negative TB test required
- Basic computer proficiency required
Physical & Work Requirements

- Ability to lift, pull, stoop, and move throughout the facility
- Ability to operate a motor vehicle (if required)
