Description:
We are seeking a dedicated Physical Therapist to serve as a trusted ally on patients’ journey toward restoring motion and optimizing their functional abilities. As a Physical Therapist on the care team, you will deliver compassionate, personalized Physical Therapy services directly in patients’ residences.
In this rewarding role, you will empower patients and their families through comprehensive education. You will guide them in implementing effective home exercise programs, and provide ongoing support to maximize recovery and quality of life. You will also collaborate closely with other healthcare professionals, offer supervision and mentorship to therapy assistants, monitor progress closely, and contribute documentation to ensure continuity of care.
Key Responsibilities:
- Provides quality care by assessing and interpreting evaluations.
- Design personalized treatment plans to restore independence and improve physical performance
- Educate patients and caregivers on therapeutic exercises and safe mobility techniques
- Monitor response to treatment and changes in condition
- Prepare accurate and timely clinical reports, progress notes, and discharge summaries
Qualifications:
- Master's degree
- At least one year experience in a Healthcare Setting
- Active Physical Therapist, PT license in Kansas and Missouri
- Valid Driver License
