Collaborates with overall practice management, monitors special populations as part of the HIV team and acts as a consultant to other Medical Providers.

Participates in Quality Improvement (QI) projects in collaboration and coordination with the HIV team.
- Assists in coordinating team-based QI projects.

Physician, Nurse Practitioner, or Physician Assistant as their primary job description.
Prior training in primary care (MD/DO residency, NP residency, or clinical experience).

Familiarity with clinical practice in Patient-Centered Medical Home model of care required.

Note: Fellow will be 1.0 FTE employed at FHCW with 0.4 FTE clinical.
