Responsibilities
- Perform patient observation and monitor vital signs to track health status and report changes promptly.
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Administer medications and manage medication records in compliance with HIPAA regulations and medical documentation standards.
- Operate Electronic Health Record (EHR) systems to document patient care accurately and efficiently.
- Support clients with dementia or Alzheimer's care, memory care needs, hospice support, and working with individuals with developmental disabilities or disabilities.
- Utilize medical terminology correctly when documenting care activities and communicating with healthcare teams.
- Use specialized equipment such as Hoyer lifts, catheters, and other assistive devices to ensure safe patient handling.
- Assist with meal preparation, cleaning, laundry, and other household tasks to promote a safe and healthy environment.
- Follow infection control protocols and maintain a clean environment to support patient health and safety.
- Provide emotional support and companionship to enhance overall quality of life for clients in home and community settings.
- Maintain compliance with all regulatory standards including HIPAA privacy rules, ensuring confidentiality of medical records and documentation.
Experience
- Previous experience working in assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated expertise in patient observation, medication administration, catheter care, and hospice or dementia care.
- Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation of patient information.
- Experience working with individuals with developmental disabilities or disabilities is a plus.
- Long-term care experience that highlights your ability to provide consistent, compassionate support over extended periods is valued.
- Knowledge of medical terminology relevant to home health care practices.
- CPR certification is required; additional certifications such as HIPAA training or hospice care certification are advantageous.
