PRINCIPAL ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS:

· Maintain professionalism and serve as a strong representative to uphold the business brand, reputation and culture.
· Attend all introduction home visits. Schedule, conduct, and document supervisor home visits, collecting client feedback on caregiver performance of duties. Address problems, as required. Review the care plan to determine relevance and accuracy. Adjust care plan based on any changes in client condition or ability. Communicate changes to all care staff involved in the case. Observe the caregiver delivering care to the client. If the caregiver is having difficulty meeting the expected level of care, provide training/education to increase/improve skill set.
· Conduct follow-up calls with clients after the first visit, every 30 days, until their 90-day reassessment, to ensure client satisfaction.
· Reassess all clients at least every 90 days and as needed, updating the care plan and notifying all involved.
· Complete organic marketing after reassessments and update all healthcare professionals involved in clients’ care via phone calls and mailing the updated care plan along with the reassessment doctor letter.
· Provide caregivers with exceptional training; at office and clients homes. Create educational opportunities for caregivers utilizing online resources, healthcare-related schools, referral sources (i.e. hospice), Life Care Navigation programs, etc. to promote and encourage professional growth.
· Build and maintain strong relationships with clients, client’s family members and caregivers through frequent communication and addressing concerns promptly.
· Independently respond to and resolve client complaints and concerns.
· Communicate with and Assist in-office Recruitment/Retention team with everything as needed; answering phones, making schedule changes, filling shifts, working shifts, interviews, orientation, etc.
· Communicate with and Assist out-office Business Development team with everything as needed; Hospital/rehab visits, assessments, marketing, etc.
· Engage in constructive problem-solving and exploration of new ideas.
· Support change and speak positively about it with employees.
· Understand the importance of each task and prioritize the workload, shifting priorities to accommodate change and demand.
· Make timely, well-informed and appropriate decisions by examining all the relevant and available facts; explore alternatives and commit to action.
· Work toward high-quality decisions that achieve organizational goals.
· Maintain absolute confidentiality of all information pertaining to employees, clients and clients’ families.
· Manage on-call evening and weekend care scheduling, as directed. Keep on-call materials current with client information, instructions and directions, current roster of care and roster of active direct care staff. Prepare and maintain the on-call log, client and employee records. Manage calls in accordance with the company policies.
· Maintain regular and predictable attendance. Attend Morning Meetings.
· Perform duties deemed appropriate by the senior management.
QUALIFICATIONS, REQUIRED JOB KNOWLEDGE AND SKILLS:
· Associate Degree in a health care related field and two years of supervisory experience in home health/care or facility is preferred.
· Social work background preferred.
· Excellent conceptual thinking skills with the capacity to analyze complex issues that are sensitive in nature.
· Excel in conflict resolution and solution implementation.
· Ability to listen and communicate clearly, fluently, diplomatically – both orally and in writing.
· Ability to remain flexible, resilient, calm and maintain a sense of humor in all situations.
· Present a well-groomed image that reflects the professionalism of the business.
· Ability to plan, organize, prioritize, delegate and accurately complete work activities within deadlines while managing interruptions.
· Work independently and proactively with minimal direction and/or supervision.
· Ability to generate goodwill for the Agency with clients, their family members and other referral sources. Demonstrate a strong commitment to client service excellence.
· Ability to lawfully work in the U.S.
Pay: From $48,000.00 per year
