Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment, such as Hoyer lifts, and ensuring safety during transfers.
- Observe and document patient conditions accurately, including changes in health status, using Electronic Health Record (EHR) systems.
- Support patients with memory care needs, including Alzheimer’s disease and dementia care, providing reassurance and maintaining a calm environment.
- Help with household chores, including cleaning, laundry, meal preparation, and ensuring a safe and hygienic environment in compliance with HIPAA regulations.
- Maintain detailed medical documentation and records to ensure continuity of care and regulatory compliance.
- Collaborate with interdisciplinary teams to deliver holistic hospice care tailored to each patient's needs in home or community settings.
Qualifications
- Must have 2+ years of experience in nursing homes, assisted living facilities, or home health and hospice environments preferred; experience working with individuals with disabilities or developmental disabilities is a plus.
- Must have reliable transportation with a valid driver license.
- Knowledge of medical terminology.
- Certification in CPR is required; additional certifications such as hospice care training or long-term care experience are advantageous.
- Ability to work independently while following established protocols; strong communication skills are essential for effective patient interaction.
- Familiarity with EMR/EHR systems for documentation; understanding of HIPAA privacy standards is necessary.
