Compensation: Hourly Set Rate. Contractors are paid their agreed-upon hourly rate strictly for submitted and approved billable hours (e.g., if you submit one hour of billable service, you are paid for one hour at your set rate).
Schedule: Flexible/Non-Traditional. Hours are driven by client needs, required nursing visits, meetings, and the on-call schedule.
Role Overview & Duties:
As a Contracted Family Living Nurse, you will identify and support the individual's preferences regarding their health decisions. While your primary focus may be Family Living, certain duties—like the on-call rotation—extend to all individuals served by the agency. Core responsibilities include:
- Home Visits: Conducting regular home visits based on the specific acuity levels of the individuals served.
- Documentation & Reporting: Completing and maintaining accurate eCHATs, Health Care Plans (HCPs), and other essential medical reports.
- Meeting Attendance: Actively attending IDT meetings and other required agency or team meetings to provide medical oversight and input.
- On-Call Rotation: Actively participating in a scheduled on-call rotation to ensure continuous health support and triage for all individuals served by the agency (not limited to Family Living).
- Health Management: Assessing, planning, monitoring, and managing health-related issues, as well as supporting health awareness and the self-management of medications and health conditions.
- Education & Communication: Providing vital health education and sharing information collaboratively among Interdisciplinary Team (IDT) members, Direct Support Professionals (DSPs), and natural supports (when requested by the individual or guardian).
- Proactive Care: Responding proactively to chronic and acute health changes or concerns to facilitate prompt access to appropriate healthcare services, coordinating care both within and beyond the DD Waiver system.
Experience & Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid, current license.
- Previous experience working within the ID/DD Waiver setting is highly preferred, though we are willing to train the right professional.
- Familiarity with Therap and iTherapyDocs is a strong plus.
- Excellent communication and interpersonal skills to effectively interact with individuals receiving services, their families, and team members.
- Exceptional attention to detail and strong organizational skills to ensure accurate documentation and overall patient safety.
