Nurse Practitioner / Physician Assistant — Mobile Primary Care

Position Summary
We are seeking a compassionate, motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile primary care team. You will manage a panel of homebound and facility-based patients, providing comprehensive primary care through in-home and on-site visits across the Metro Detroit area.
Responsibilities
- Conduct comprehensive primary care visits in patients' homes, SNFs, and assisted living facilities
- Perform history and physical exams, diagnose and manage acute and chronic conditions
- Develop and manage individualized care plans, including medication management
- Order, perform, and interpret diagnostic tests as clinically indicated
- Coordinate care with specialists, hospitals, home health agencies, and facility staff
- Maintain timely, accurate documentation in the practice's EMR system
- Communicate effectively with patients, families, and caregivers regarding care plans
- Participate in on-call rotation as needed
- Ensure compliance with all applicable state and federal regulations governing home-based and facility-based care
Qualifications
- Active, unrestricted Michigan license as a Nurse Practitioner or Physician Assistant
- Current national certification (ANCC, AANP, or NCCPA as applicable)
- Active DEA registration (or ability to obtain)
- Experience in primary care, geriatrics, home-based care, or a related field preferred
- Valid driver's license and reliable transportation (mileage/travel required between visit locations)
- Strong clinical judgment and ability to work independently in varied care settings
- Excellent communication and interpersonal skills with patients, families, and facility staff
- Comfort with EMR documentation and mobile/point-of-care technology
- Experience working with SNF or assisted living populations a plus
