Job Summary:
The Level 2 Group Home Direct Care Staff is responsible for providing daily supervision, support, and care for youth ages 10–17 in a structured residential setting. This position ensures the safety, well-being, and development of residents by implementing individualized service plans, teaching life skills, modeling positive behaviors, and fostering a supportive, trauma-informed environment. Staff will work as part of a multidisciplinary team to promote stability, skill development, and successful transition to the community.
Key Responsibilities:Resident Care & Supervision
- Provide 24/7 care coverage according to scheduled shifts, maintaining required staff-to-youth ratios.
- Supervise residents in all daily activities, including meals, hygiene, recreation, school preparation, and bedtime routines.
- Ensure compliance with all Level 2 group home rules, policies, and behavioral expectations.
- Use trauma-informed care and de-escalation techniques to address behavioral challenges and crisis situations.
Program Implementation & Support
- Implement each resident’s individualized service plan (ISP) and document progress toward goals.
- Facilitate life skills training, including communication, problem-solving, conflict resolution, and independent living skills.
- Assist residents in attending school, therapy, medical appointments, and extracurricular activities.
- Engage youth in structured activities and encourage participation in community events.
Documentation & Compliance
- Maintain accurate, timely, and detailed documentation of resident behavior, incidents, medication administration, and daily logs.
- Report any incidents, safety concerns, or suspected abuse/neglect immediately, following mandated reporting laws.
- Adhere to North Carolina Department of Health and Human Services (DHHS) and Medicaid regulatory requirements for Level 2 group homes.
Collaboration & Communication
- Work collaboratively with supervisors, case managers, therapists, and other service providers.
- Communicate regularly with residents’ legal guardians, social workers, and other involved parties, as directed by the supervisor.
- Attend staff meetings, training sessions, and supervision as required.
Qualifications:
- High school diploma or GED required; Associate’s or Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred.
- Minimum of one (1) year of experience working with youth in a residential, mental health, or child welfare setting.
- Completion of required pre-service and ongoing training, including CPR/First Aid, Crisis Intervention, and Medication Administration.
- Must pass criminal background check, driving record check, and drug screening.
- Valid North Carolina driver’s license and reliable transportation.

Working Conditions:
- Work in a residential facility with rotating shifts, including evenings, weekends, and holidays.
- May be required to transport youth to appointments or activities.
- Ability to lift up to 25 pounds and participate in physical activities with residents.

Pay: $17.00 - $19.00 per hour
