Duties
- Conduct thorough patient assessments, including medical history review and physical examinations, to determine health status and develop personalized care plans
- Manage acute and chronic conditions across a broad spectrum of primary care needs, utilizing evidence-based practices
- Order, interpret, and discuss diagnostic tests such as laboratory results, imaging reports, and other relevant diagnostics to inform treatment decisions
- Provide preventive care services including immunizations, health screenings, counseling on lifestyle modifications, and health coaching
- Collaborate with an integrated care team — including behavioral health and care coordination — on patient care, follow-up, and referrals
- Maintain accurate, timely documentation of all clinical encounters in our EHR system
- Educate patients on disease management, medication adherence, and wellness strategies to empower informed health choices
Requirements
- Current WV license as an MD, DO, NP, or PA (or eligibility to obtain)
- Active, unrestricted DEA registration
- Familiarity with EHR platforms for documentation and patient data management
- Ability to perform comprehensive physical examinations and patient assessments efficiently in fast-paced environments
- Excellent communication skills for patient education and interdisciplinary collaboration
- A genuine interest in community and rural health
