Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems
- Support medication administration and ensure proper documentation in compliance with HIPAA regulations
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory-related conditions, including memory care techniques
- Perform patient observation to detect changes in condition or behavior and communicate findings promptly to healthcare professionals
- Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice care support as needed
- Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, and meal preparation
- Document all patient interactions and medical records thoroughly using Electronic Medical Records (EMR) systems to ensure accurate medical documentation
- Support clients with home & community care needs by helping with errands or transportation when appropriate
- Follow all safety protocols including CPR certification requirements and proper use of medical equipment
Requirements
- Proven experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience preferred
- Strong knowledge of medical terminology, patient care procedures, and caregiving best practices
- Certification in CPR is required; additional certifications such as hospice care or dementia training are a plus
- Experience working with individuals with developmental disabilities or disabilities requiring specialized support is highly desirable
- Ability to perform physically demanding tasks including lifting with Hoyer lifts and assisting with mobility needs
- Familiarity with EMR/EHR systems for medical documentation and record keeping
- Excellent communication skills to effectively collaborate with patients, families, and healthcare teams
- Commitment to maintaining patient privacy and confidentiality in accordance with HIPAA standards
- Valid driver’s license and reliable transportation are preferred for home & community visits

Pay: $15.00 - $17.00 per hour
