Our LVNs are the clinical backbone of our care model. You'll play a hands-on role in ensuring the health, safety, and dignity of our clients — while mentoring and supporting the caregiving team that delivers care every day. WHAT YOU'LL DO
Clinical Care:
- Provide in-home care ADLs when necessary
- Communicate with physicians to obtain and clarify medication orders
- Keep clients and families informed and educated on medication requirements and any changes to care
Client Assessment & Care Planning:
- Conduct nursing assessments for all new and existing clients
- Collaborate closely with the Executive Director on developing and updating individualized care plans
- Perform 90-day RN supervisory reviews and visit documentation
- Oversee hospice care coordination and maintain active communication with outside hospice agencies
Caregiver Training & Supervision:
- Clinical orientation for new hire caregivers
- Train caregivers on all delegated nursing tasks, ensuring competency and compliance
- Be available to provide real-time supervision, guidance, and clinical direction to caregivers on duty
- Serve as a clinical mentor and resource for the field caregiving team
Compliance & Documentation:
- Maintain current knowledge of California regulations governing LVN practice, nursing delegation, and in-home care
- Ensure all state administrative rules governing in-home care are met at all times
WHAT WE'RE LOOKING FOR
- Current, unencumbered California LVN license in good standing
- Prior experience in home care (in-home care or home health) strongly preferred
- Experience with nursing delegation to unlicensed caregiving staff
- Experience working with the senior population
- Prior experience coaching, training, or supervising caregivers or CNAs
- Strong verbal and written communication skills — you'll interface with clients, families, physicians, and field staff daily
- Basic computer skills, including word processing, email, and electronic documentation platforms
- Valid California driver's license and reliable transportation required
- Bilingual English/Spanish a strong plus
- Flexible schedule with willingness to support after-hours clinical needs as they arise
- Ability to lift 50 lbs. using safe, ergonomic movements

- Pay:$40/hour depending on experience and hours
