Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Your expertise in patient assessment, medication administration, and clinical procedures will ensure optimal health outcomes for diverse patient populations. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments.
- Manage complex care needs such as wound dressings, catheterization, tube feeding, ventilator management, and pain control strategies.
- Document patient care activities thoroughly using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems while ensuring compliance with HIPAA regulations.
- Collaborate with physicians, residential managers, and interdisciplinary teams to develop discharge plans and coordinate ongoing care for cleints.
- Provide education and support to staff on health coaching, medication management, and post-acute care instructions.
- Participate in on-call rotation with company RN.
Skills
- Proven experience working with diverse patient populations in settings such as hospitals, nursing homes, or home care environments.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and infection control protocols.
- Familiarity with EMR/EHR systems.
- Ability to handle emergency situations efficiently with critical thinking skills.
- Experience working with individuals with developmental disabilities or behavioral health needs; knowledge of dementia or memory care is a plus.
