Practice Overview
Ormond Family Medical Center is an independently owned and operated, nurse practitioner-owned family practice located in Ormond Beach, Florida. We are seeking a highly motivated, experienced Autonomous Nurse Practitioner who is passionate about high-quality primary care, value-based medicine, patient relationships, and independent practice.
Our practice operates within a value-based care model, allowing providers to focus on thoughtful, patient-centered care rather than high-volume patient turnover. This position offers a low daily patient load, supportive clinical environment, competitive compensation, paid time off, holiday pay, and opportunity for performance-based bonuses.
Position Summary
The Autonomous Nurse Practitioner will provide comprehensive primary care services to adult and/or family medicine patients in an outpatient setting. The ideal candidate will have strong clinical judgment, excellent communication skills, comfort managing chronic disease, and the ability to practice independently within the scope of Florida autonomous practice privileges.
Prior experience in critical care, internal medicine, emergency medicine, hospital medicine, or complex chronic disease management is a plus.
Minimum Qualifications
- Active and unrestricted Florida APRN license & RN license
- Current Autonomous Practice privileges through the Florida Board of Nursing
- Minimum 3–5 years of clinical nurse practitioner experience
- Family practice, internal medicine, urgent care, critical care, or complex chronic disease experience preferred
- Current DEA registration or eligibility
- Current BLS certification
- Strong clinical assessment, diagnostic, and documentation skills
- Ability to manage chronic conditions including diabetes, hypertension, hyperlipidemia, obesity, thyroid disease, COPD/asthma, anxiety/depression, and preventive care
- Experience with Medicare Advantage, value-based care, or ACO models preferred but not required
- experience with e clinical works a plus
Key Responsibilities
- Provide independent primary care evaluation, diagnosis, treatment, and follow-up
- Manage acute and chronic medical conditions
- Perform annual wellness visits, chronic care visits, transitional care visits, and preventive health visits
- Order and interpret labs, imaging, and diagnostic studies
- Prescribe medications within scope of practice and applicable regulations
- Develop individualized care plans for patients with complex medical needs
- Participate in value-based care quality initiatives
- Support patient retention, care coordination, and population health goals
- Complete accurate, timely, and compliant medical documentation
- Collaborate with specialists, home health, hospitals, pharmacies, and care management teams
- Educate patients and families regarding treatment plans, prevention, lifestyle, and medication adherence
- Maintain professional standards, patient confidentiality, and compliance with all applicable laws and regulations
Compensation and Benefits
Standard Employment Track
Salary range: $120,000–$135,000 annually, depending on experience, qualifications, productivity, and negotiated terms.

Optional Fellowship / Mentorship Track
Independent Practice Ownership Fellowship Program
Ormond Family Medical Center also offers a unique fellowship and mentorship opportunity for an Autonomous Nurse Practitioner who desires to eventually open, own, and operate an independent medical practice.
This program is designed for a motivated APRN seeking advanced professional development in both clinical independence and healthcare business ownership.
Fellowship Purpose
The purpose of the Fellowship and Mentorship Program is to provide advanced professional training, mentorship, and practical business ownership experience to an Autonomous Nurse Practitioner licensed in the State of Florida who desires to transition into ownership and operation of an independent medical practice.
The Fellow will receive hands-on education and mentorship from an experienced Nurse Practitioner practice owner, educator, and advocate for Nurse Practitioner business ownership.
Required Commitment
Participation in the Fellowship/Mentorship Program requires a minimum 18-month contractual commitment.
