This position provides care throughout the Central Valley, including:
- Fresno
- Clovis
- Madera
- Sanger
- Surrounding communities
Position Summary
As a Per Diem Medical Social Worker, you'll provide psychosocial assessments, counseling, education, and resource coordination to home health patients and their families. Working collaboratively with physicians, nurses, therapists, and other members of the care team, you'll help patients overcome barriers that may affect their health, independence, and quality of life.
What You'll Do
- Conduct comprehensive psychosocial assessments in patients' homes.
- Identify emotional, financial, environmental, and social barriers to care.
- Develop individualized plans to support patient goals and improve outcomes.
- Connect patients and families with community resources and support services.
- Provide counseling, education, and emotional support.
- Collaborate with physicians and the interdisciplinary care team.
- Participate in case conferences and care planning.
- Document patient visits and interventions accurately in WellSky/Kinnser.
- Maintain compliance with Medicare, state, and agency regulations.
Qualifications
- Master's Degree in Social Work (MSW) from a CSWE-accredited program.
- Minimum one (1) year of healthcare experience preferred.
- Home Health or Hospice experience is a plus.
- Knowledge of community resources and care coordination.
- Excellent communication, organization, and interpersonal skills.
- Current California driver's license with reliable transportation and current auto insurance.
- Current CPR/BLS certification (or ability to obtain).
