Provides moderately complex (journey-level) case management work in the field of aging and senior care services.
Work involves:

Visiting nursing facilities to complete the Pre-admission-Screening-Resident-Review (PASRR) assessment required by the Department of Aging and Disability Services (DADS).

Carrying a caseload of those nursing home residents that are eligible for Pecan Valley Centers’ community services as determined by the PASRR assessment and by the choice of the resident.

Identifying and documenting the service needs of individuals by developing a person directed treatment plan

Coordinate and conduct treatment team staffing’s

Coordinate the delivery of necessary services within required service delivery time frames.

Must drive to nursing facilities within the Pecan Valley six county service areas of Erath, Hood, Johnson, Palo Pinto, Parker and Somervell counties.

Works independently

Must complete documentation within strict time frames.

Bachelor’s degree in a field of human/social services required.

1 year experience working in Individuals with Developmental Disabilities.
- Knowledge and ability to perform basic computer skills a necessity.

Must be at least 18 years of age

Have a Texas driver’s license

Have a good driving record for the past three years

Have a valid auto insurance card.
