Responsibilities:
Serve as a liaison between patients, families, and healthcare providers to ensure seamless communication and care coordination
Develop and maintain strong relationships with patients, families, and healthcare partners to ensure high-quality care and satisfaction
Assist with patient intake, registration, and scheduling to ensure efficient and timely care delivery
Monitor and report patient flow and throughput to optimize care delivery and reduce wait times
Collaborate with care teams to identify and address patient needs, including medical, emotional, and spiritual support
Provide education and support to patients and families on treatment options, medication management, and disease prevention
Participate in quality improvement initiatives to continually improve patient outcomes and satisfaction
Requirements:
High school diploma or equivalent required; degree in a related field (e.g., healthcare, social work) preferred
