We are seeking a compassionate and dedicated Registered Nurse to join our home health care team on a contract basis. The mission of this role is to provide exceptional, patient-centered care directly in the home environment, ensuring that individuals can recover or manage chronic conditions with dignity and clinical excellence. You will serve as a vital link between patients and their physicians, managing complex care plans and promoting positive health outcomes through evidence-based interventions.
Your day-to-day responsibilities will include conducting comprehensive physical assessments, managing medication administration, performing wound care, and providing vital education to patients and their family caregivers. You will maintain meticulous documentation of patient progress in compliance with clinical standards and coordinate with a multidisciplinary team to adjust care plans as needed. This role requires traveling to various patient residences within the assigned territory and managing a daily schedule with autonomy.

Active, unrestricted Registered Nurse (RN) license in the state of practice

Minimum of 1-2 years of clinical nursing experience, preferably in home health or acute care

Current Basic Life Support (BLS) certification from the American Heart Association

Possession of a valid driver's license and a reliable, insured vehicle for daily travel

Proficiency in Electronic Medical Record (EMR) systems and mobile documentation technology
