In this vital role, you will assess individual needs, develop personalized care plans, and deliver evidence-based physiotherapy interventions in the ALF setting.
Duties
- Utilize WebPT EMR
- Deliver hands-on therapy sessions in the ALF setting at fabricated wellness centers or homes of the client allowing functional application.
- Manage referral/discharge planning processes by coordinating with multidisciplinary teams to ensure health and wellness at home
- Monitor patient progress regularly, adjusting treatment strategies based on physiology responses and clinical observations.
- Integrate with ALF wellness teams/providers with focus on health/wellness and patient outcomes.
Qualifications
- Valid Arizona licensure or certification as a licensed Physical Therapy Assistant
- Strong knowledge of physiology, anatomy, medical terminology, and specialized manual therapies.
- Demonstrated ability to implement care plans for vulnerable ALF population.
- Excellent communication skills to collaborate with healthcare teams and educate patients effectively.
