POSITION SUMMARY
The Registered Nurse (RN) is responsible for providing skilled nursing services, performing comprehensive assessments, developing individualized care plans, coordinating care, and ensuring the delivery of safe, effective, and compassionate care in the home setting.
The ideal candidate will possess strong clinical judgment, exceptional communication skills, and experience caring for medically complex patients in the home health and private duty setting.

ESSENTIAL DUTIES & RESPONSIBILITIES
- Conduct comprehensive patient assessments and ongoing evaluations
- Complete Home Health Start of Care (SOC), Recertification, Resumption of Care (ROC), Transfer, Discharge, and OASIS documentation
- Develop, implement, and update individualized patient care plans
- Administer medications, treatments, and skilled nursing interventions as ordered
- Monitor patient status and identify changes in condition
- Communicate effectively with physicians, therapists, caregivers, and interdisciplinary team members
- Provide patient and family education regarding disease management, medications, safety, and treatment plans
- Maintain accurate and timely clinical documentation
- Participate in care coordination and case management activities
- Ensure compliance with agency policies, state regulations, and professional standards of practice
- Promote patient safety, independence, dignity, and quality of life

REQUIRED QUALIFICATIONS
- Active Indiana Registered Nurse (RN) License
- Minimum of two (2) years of Home Health experience required
- Home Health Start-of-Care (SOC) experience required
- OASIS documentation experience required
- Recertification experience required
- Current CPR Certification
- Strong assessment, communication, and clinical documentation skills
- Ability to work independently and manage patient care effectively
- Reliable transportation and ability to travel as needed
PREFERRED QUALIFICATIONS
- Axxess Home Health experience preferred
- Experience caring for medically complex patients
- Experience with spinal cord injury and neurological conditions
- Experience with tracheostomy and respiratory care
- Private Duty Nursing experience
IMPORTANT:
Candidates without Home Health Start-of-Care (SOC), Recertification, and OASIS experience will not be considered.
