We are seeking a Full-time Psychiatric Nurse to provide and assist team members in coordinating the behavioral and physical health care needs of program participants.
Other Responsibilities, but not all-inclusive:
- Complete health assessments for participants in the program.
- Maintain a record of medical and behavioral health history and ongoing medical and treatment records. Review all charts at least once a month to ensure medical needs are met and that documentation meets applicable standards.
- Consult with and assist team members regarding the development and implementation of the health care needs and goals.
- Negotiate the health care responsibilities of participants with provider agencies.
- Coordinate with team members’ routine medical, vision, lab, psychiatric, and dental appointments. Ensure a mechanism for reporting and follow-up on results. May accompany the participant to an appointment when determined by the nurse in consultation with the team, and facilitate communication between the primary care physician (PCP) and psychiatrist.
- Attend team meetings regularly to review medications, emergency intervention plans, and response to treatment, and to review health care needs, participants' response to treatment, and coordinate scheduling appointments.
- Monitor participants' medication. Verify insurance coverage by calling the appropriate insurance provider and pharmacies as needed. Assist the psychiatrist in completing prior authorizations if needed.
- Participate in discharge planning for participants from acute or extended care facilities or forensic settings to provide continuity of medical, substance abuse, and psychiatric care.
Qualifications:
- Licensed RN/LPN in the State of PA.
- Prior experience in community mental health and/or substance abuse treatment settings.
- Knowledge of psychiatric clinical and psychosocial rehabilitation practices.
- Excellent communication and interpersonal skills.
- Good writing and computer skills for documenting in an electronic health record.
- Team-oriented.
- Detail-oriented with strong organizational and analytical skills.
- Must be available for scheduled emergency on-call service.
- Must possess a valid driver’s license with an acceptable driving record.
Scope:
- Frequent talking and listening.
- Will require crisis intervention in emergencies, including managing aggression.
- Must be available for local travel
- Scheduling flexibility and possible on-call work are necessary.
- Will be required to report to work during emergencies, including inclement weather.
