Job Summary
The Physical Therapist (PT) provides intermittent skilled physical therapy services to patients in
their place of residence in accordance with physician orders and the established plan of care.
Services are delivered under the direction and supervision of the Clinical Manager, Nursing
Supervisor, and/or Therapy Supervisor.
Qualifications
- Graduate of an accredited Physical Therapy program approved by:
- The American Physical Therapy Association (APTA), or
- The Council of Medical Education of the American Medical
Association
- Current Physical Therapy license in the state(s) of practice
- Current CPR certification
- Ability to work independently and exercise sound clinical judgment
- Alignment with the mission, values, and goals of Prestige Therapies
Reporting Structure
Reports to the Clinical Manager, Nursing Supervisor, and/or Therapy Supervisor
Key Responsibilities

- Evaluate patient functional status and assist in developing and updating
- Perform skilled physical therapy treatments as ordered by the physician Complete initial evaluations, reassessments, and discharge summaries
- Prepare and submit clinical documentation, progress notes, and reports
in a timely manner
- Complete OASIS assessments, reassessments, resumptions of care,
transfers, discharges, and significant change in condition documentation
per agency timelines
- Communicate patient progress and treatment changes to physicians and
interdisciplinary care team members
- Educate patients and family members on home exercise programs and
activities of daily living
- Supervise Physical Therapy Assistants (PTAs) and Home Health Aides
(HHAs) as required
- Participate in patient care conferences, staff meetings, and in-service
education
- Adhere to agency policies, procedures, and professional standards of
care
- Maintain patient confidentiality and safeguard all protected health
information
- Promptly report patient incidents, variances, or complaints to supervisory
staff
- Demonstrate professional appearance, time management, and timely
documentation practices
- Maintain a clean, organized, and safe work environment
- Perform additional duties as assigned
Working Environment
- Field-based position providing care in patient homes
- Occasional work in agency office as needed
- Frequent travel between patient residences
