Care Manager — Permanent Supportive Housing Newbridge Place | Lodi, OH | Full-Time | $18–$20/hour
Who We Are
Newbridge Place, a non-profit has provided permanent supportive housing for adults with co-occurring mental health and substance use disorders for over 28 years — with a mission to serve low-income veterans. Our 26-bed licensed residential program in Lodi, Ohio serves veterans and non-veterans who have often exhausted every other option. We are looking for a Care Manager who leads with empathy, keeps their head when things get complicated, and takes genuine ownership of the people in their care.
What You Will Do
Your core responsibility is ensuring that each resident receive individualized care that reflects their actual needs. That means:
- Developing, implementing, and regularly updating individualized care plans in collaboration with residents, families, and healthcare providers
- Advocating for resident rights and ensuring compliance with ODMHAS licensing standards and applicable regulations
- Supervising and documenting residents' self-administration of medications; coordinating timely prescription refills and ensuring accuracy
- Transporting residents to medical appointments in the Cleveland and Mansfield areas (a reliable vehicle and valid Ohio driver's license are required)
- Coordinating with outside providers — physicians, social workers, home care services — and maintaining accurate, up-to-date documentation
- Conducting ongoing assessments and adjusting care plans as residents' needs change
- Organizing activities that build social connection and support resident well-being
- Maintaining a clean, safe, and dignified living environment
This is a hands-on, in-person position. You will be the consistent point of contact for residents who rely on you for both clinical coordination and human connection.
What We're Looking For
Required:
- Minimum 2 years of experience in caregiving, care coordination, case management, or a closely related role
- Valid Ohio driver's license and reliable transportation
- Strong written and verbal communication skills
- Ability to maintain strict confidentiality and professional boundaries
- Ability to commute to or relocate to Lodi, OH 44254
Strongly Preferred:
- BA degree or equivalent
- Experience in group home, supported housing, or community mental health settings
- Familiarity with psychiatric medications and medication management protocols
- Background in social work, psychology, or a related field
- Understanding of resident rights, trauma-informed care, and person-centered planning
- The right person for this roleis calm under pressure, organized enough to manage multiple residents' care plans simultaneously, and committed enough to stay — this is not a stepping stone job, it is the job.
Schedule
- Monday–Friday, 7:30 am-3:30pm day shift (8-hour shifts), with on-call rotation and occasional weekend coverage as needed.
