Responsibilities
- Perform thorough patient assessments, including vital signs, physical examinations, and detailed medical histories to inform individualized care plans.
- Administer medications and treatments accurately, including IV infusions, injections, catheter care, and wound management, adhering strictly to safety protocols.
- Monitor patients’ conditions continuously, utilizing advanced EMR (Electronic Medical Records) systems such as Point Click Care (PCC) to document all care activities precisely.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and post-acute care services tailored to each resident’s needs.
- Provide specialized care for residents with dementia or Alzheimer’s disease, including memory care strategies and behavioral health support.
- Ensure compliance with HIPAA regulations and infection control standards while maintaining high-quality patient care standards across all settings.
Requirements
- Valid licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Nevada.
- Proven experience in long-term care facilities or skilled nursing environments; experience with geriatrics highly preferred.
- Familiarity with EMR/EHR systems such Point Click Care; proficiency in medical documentation is essential.
- Expertise in medication administration, IV infusion therapy, catheterization procedures, and basic life support certification.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Demonstrated ability to work under pressure within fast-paced environments such as emergency medicine or hospital medicine units.
