Physician Assistant (PA-C) — Skilled Nursing Facility Rounding | Part-Time

About the Role
A physician-led practice serving skilled nursing facilities in the greater Sacramento area is seeking an experienced Physician Assistant to join our rounding team. You will manage a defined panel of long-term care and post-acute patients across partner facilities, working under a collaborative practice agreement with our supervising physician.
This is a strong fit for a PA who wants autonomy, predictable patient volume, and no hospital call schedule.
What You'll Do
- Round on an assigned SNF panel, performing subsequent care visits, admission and discharge evaluations, and acute change-of-condition assessments
- Manage chronic conditions common to the long-term care population — CHF, COPD, diabetes, dementia, CKD, wound care, polypharmacy
- Complete accurate, timely documentation and coding within the same business day
- Coordinate with facility nursing staff, DONs, families, and consultants
- Participate in care plan meetings and support goals-of-care conversations
- Respond to facility calls during scheduled hours
What We're Looking For
Required
- Current, unrestricted California Physician Assistant license
- NCCPA certification (PA-C)
- Active DEA registration (or eligible to obtain)
- Medicare enrollment / PECOS-eligible, or willingness to enroll
- Reliable transportation and valid CA driver's license — travel between facilities is required
- Comfort working independently with physician support available by phone
Preferred
- 1+ year of geriatric, SNF, long-term care, or post-acute experience
- Familiarity with SNF billing and documentation requirements (99304–99310, 99315–99316)
- Experience with point-of-care EHR documentation
- Bilingual a plus
