Pediatric Asthma Care Manager

Position Summary
Alger Pediatrics is seeking a compassionate, organized, and motivated Pediatric Care Manager who will specialize in asthma disease management, to join our growing team. This full-time, in-person position plays a vital role in improving the health and quality of life of children with asthma through education, care coordination, and ongoing support. The ideal candidate is passionate about pediatric care, excels at building relationships with families, and is committed to helping patients achieve better asthma control.
Responsibilities
- Provide individualized asthma education to patients and families, including medication administration, inhaler and spacer technique, and trigger avoidance.
- Develop, implement, and update personalized Asthma Action Plans in collaboration with providers.
- Monitor asthma patients through follow-up phone calls, office visits, and electronic communication.
- Coordinate care with pediatric providers, specialists, schools, pharmacies, and community resources.
- Review patient records to identify children who would benefit from asthma management services.
- Assist with medication prior authorizations and prescription refill requests related to asthma care.
- Document all patient interactions accurately in the electronic medical record.
- Participate in quality improvement initiatives focused on pediatric asthma outcomes.
- Collaborate with physicians, nurses, and clinical staff to deliver exceptional patient-centered care.
Qualifications
- Current RN, Respiratory Therapist (RRT/CRT), or other appropriately credentialed individuals. This individual will need to be certified in care management if not already achieved. On-site training available.
- Experience in pediatrics, pulmonary care, or chronic disease management preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and patient education abilities.
- Ability to work independently while collaborating effectively with a multidisciplinary team.
- Proficiency in Microsoft Office and electronic medical records, with excellent typing skills.
Schedule
- Full-time, 32 hours a week with a minimum of 3-4 days in office.
- In-person
- Monday through Friday
- Typical office hours (9am to 5pm) with occasional flexibility based on patient needs
