The South Carolina Primary Health Care Association (SCPHCA) is seeking a compassionate, energetic, and community-focused Outreach Worker to join our South Carolina Agricultural Worker Health Program (AgWHP). In this rewarding role, you'll help connect agricultural workers and their families with essential healthcare services while making a lasting impact on the health and well-being of one of our state's most vital workforces.

As an Outreach Worker, you will become a trusted advocate by:
- Connecting farmworkers and their families with healthcare services.
- Providing health education and resources.
- Assisting with transportation and care coordination.
- Building relationships with growers, community organizations, and healthcare providers.
- Helping improve health outcomes for agricultural worker populations across South Carolina.

What You'll Do
As a valued member of our AgWHP team, you will:
- Conduct outreach throughout the Midlands, Pee Dee, Upstate, and Lowcountry regions of South Carolina.
- Educate agricultural workers and their families about available healthcare services.
- Determine program eligibility and enroll qualified clients.
- Provide bilingual (English/Spanish) interpretation and translation services.
- Coordinate transportation to medical appointments.
- Conduct health education presentations.
- Perform basic health needs assessments.
- Connect clients with healthcare providers and follow up to ensure continuity of care.
- Assist with mobile health clinics and community outreach events.
- Maintain accurate documentation and enter data into our electronic health system.
- Support program initiatives and administrative activities that improve access to care.

Qualifications
Required
- Associate's degree with at least one (1) year of experience in the public healthcare system OR
- High School Diploma/GED with two (2) or more years of public healthcare experience
- Experience working with agricultural workers/ farmworker communities
- Bilingual proficiency in English and Spanish (written and verbal)
- Strong customer service and communication skills
- Excellent organizational and time management abilities
- Proficiency with Microsoft Office (Word, Excel, Outlook, PowerPoint)

Pay: $40,233.00 - $45,480.00 per year
