Leeway is seeking a Per Diem Physical Therapist Assistant (PTA) to support our rehabilitation team in providing high-quality, resident-centered care. This position is ideal for a motivated, compassionate PTA who is committed to helping residents achieve their highest level of mobility and independence.

Key Responsibilities
- Provide physical therapy treatments to residents under the direction and supervision of a licensed Physical Therapist
- Assist with implementing individualized plans of care focused on improving strength, mobility, balance, and functional independence
- Monitor resident response to treatment and report progress or concerns to the supervising Physical Therapist
- Document daily treatment notes and participation in accordance with facility policies and regulatory requirements
- Collaborate with nursing, therapy, and interdisciplinary team members
- Educate residents and caregivers on safe mobility techniques and therapeutic exercises
- Adhere to all safety, infection control, and compliance standards
Qualifications
- Graduate of an accredited Physical Therapist Assistant program
- Current Connecticut PTA license (required)
- Skilled nursing or long-term care experience preferred, but not required
- Strong communication, documentation, and teamwork skills
- Ability to follow established plans of care and provide compassionate resident support
