Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by taking vital signs, observing symptoms, and reporting changes to healthcare professionals, as needed
- Medication reminders for Clients
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking, if needed
- Maintain a clean and safe living environment through regular cleaning and organization tasks
- Support clients with mobility using equipment such as Hoyer lifts or assisting with transfers safely, as needed
- Document care activities, and communicate effectively with healthcare teams and family members
Skills
- Proven experience in home & community care or assisted living environments
- Knowledge of patient observation, dementia care, Alzheimer’s care, and memory care techniques
- Ability to perform CPR and First Aid confidently; certification required
- Familiarity with working with individuals with developmental disabilities or autism spectrum disorder, preferred
- Strong skills in patient monitoring, and vital signs assessment
- Experience with hospice care, long-term care, or nursing home settings is advantageous
- Ability to lift heavy objects safely and assist with physical transfers using appropriate equipment
- Excellent communication skills for working collaboratively within multidisciplinary teams and supporting residents’ social needs
