Job Summary
We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, delivering high-quality patient care directly in the community. Duties
- Conduct thorough patient assessments including vital signs, physical examinations, and health histories to develop tailored care plans.
- Administer medications, injections, IV infusions, and perform catheterizations following strict protocols to ensure safety and efficacy.
- Provide specialized care for individuals with developmental disabilities, dementia, Alzheimer's disease, memory care needs, and those requiring hospice or palliative support.
- Assist with complex procedures such as tube feeding, wound suturing, sterile processing, and respiratory support including ventilator management.
- Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth while adhering to HIPAA regulations.
- Educate patients and their families on health coaching, medication management, discharge planning, and chronic disease management strategies.
- Coordinate with healthcare providers for diagnostic evaluations like medical imaging or laboratory testing; facilitate specimen collection and processing when necessary.
Skills
- Proven experience with Epic EMR systems or similar EHR platforms; familiarity with ICD-9/10 coding for accurate documentation.
- Strong background in critical care environments such as ICU, trauma centers (Level I & II), or emergency rooms; including acute pain management and infectious disease care.
- Extensive knowledge of geriatrics, dementia care (including Alzheimer's), hospice services, and long-term care settings such as nursing homes or assisted living facilities.
- Skilled in patient assessment techniques including vital signs measurement, anatomy/physiology knowledge, airway management, and IV insertion/injection administration.
- Ability to perform complex procedures like spinal taps, gastric lavage, dialysis support, or PICU interventions with precision and aseptic technique.
- Excellent communication skills for triage assessments, case management coordination, teaching hospital experience preferred for complex cases involving medical-surgical or pediatric patients.
- Working knowledge of ICD coding (ICD-9/10), CPT coding for procedures like botulinum toxin treatments or dermatological fillers; familiarity with medical terminology is essential.
