REGISTERED NURSE--HOME HEALTH/HOSPICE 7-on/7-off On-Call

This position would join a pool of on call nurses to share the load. You will provide care on a 7-on/7-off schedule.
ON CALL RN BENEFITS

ON CALL NURSE ESSENTIAL FUNCTIONS
· Takes call from 5:00pm to 8:30am M-Th, and Friday 5:00 pm to Monday 8:30am.
· Field/triage patient/family calls and concerns
· Conducts routine visits and/or admit visits with home health or hospice patients
· Assures thorough team communication regarding patient status at shift end
· Provides community education to potential patients and their families
ON CALL RN QUALIFICATIONS
· Cheerful customer service oriented.
· Current Utah RN license (required).
· Experience in the home health and hospice settings.
· Excellent availability and response time to calls.
· Demonstrated ability to triage and apply critical thinking skills.
· Knowledge of and ability to navigate electronic medical records. HomeCare HomeBase experience a plus.
· Reliable transportation is required.
