At Encompass Community Supports, our full-time employees work 37.5 hours weekly.
Our Licensed Practical Nurse (LPN), provides skilled nursing services, monitoring of clients’ health status, monitor direct support staff’s follow through of physician’s orders, medical and medication administration protocols, work is performed under regular supervision, physician’s orders and some tasks delegated by RN.
ESSENTIAL FUNCTIONS:
- Provides nursing support for outpatient behavioral health and substance use disorder services.
- Performs patient data collection, medication reconciliation, vital signs, specimen collection, and other clinical tasks within the LPN scope of practice. Documents observations and promptly reports changes in patient condition to the RN or licensed provider
- Assists psychiatric providers and medical providers during patient visits and medication clinics.
- Coordinates medication-assisted treatment (MAT), including oral and long-acting injectable medications as directed by providers.
- Monitors patients for medication side effects, adverse reactions, and withdrawal symptoms, promptly communicating concerns to the RN or provider.
- Coordinates laboratory testing, prior authorizations, prescription refills, and pharmacy communication.
- Reviews medication histories and verifies medication accuracy within the electronic health record.
- Consults with physicians, nurse practitioners, RNs, therapists, case managers, and referral sources regarding patient treatment plans.
- Provides patient education regarding medications, treatment adherence, wellness, chronic disease management, and preventative healthcare.
- Coordinates referrals for specialty care, primary care, laboratory services, imaging, and community resources.
- Tracks completion of annual physicals, preventive screenings, vaccinations, and required laboratory monitoring.
- Ensures accurate and timely nursing documentation in the electronic health record.
- Assists with quality improvement activities, chart audits, and compliance monitoring.
- Maintains compliance with DBHDS, Medicaid, DEA, HIPAA, CARF, and agency policies.
- Participates in interdisciplinary treatment team meetings
OTHER FUNCTIONS:
- Completes Human Resources Training forms.
- Attends training & maintains certifications as required by Agency and regulatory guidelines from licensing entities.
REQUIRED EDUCATION AND EXPERIENCE:
- Licensed Practical Nurse allowed to practice in the Commonwealth of Virginia.
- 1 or more years of experience in nursing, preferably in a long-term care setting and / or with people who have a developmental disability.
KNOWLEDGE, SKILLS AND ABILITIES:
Thorough knowledge of the principles, standards, practices and techniques of professional nursing; thorough knowledge of the dangers inherent in and the precautions to be taken in the use of narcotics and other medications; thorough knowledge of the treatment, diagnostic, immunization and related medical services for clients; thorough knowledge of the rules, regulations, procedures and policies pertaining to nursing services of the agency; ability to accurately maintain patient medical records and prepare reports; ability to establish and maintain effective working relationships with clients, RN, physicians and associates; knowledge of MS Office, to include Word and Excel.
PHYSICAL REQUIREMENTS:
This work requires light physical effort working almost exclusively with lightweight materials (up to 25 pounds). The worker may be exposed to bloodborne pathogens and may be required to wear specialized personal protective equipment.
