Key Responsibilities
- Evaluate patients and develop individualized treatment plans based on clinical findings and functional goals.
- Provide skilled physical therapy services to improve mobility, strength, balance, coordination, and endurance.
- Assess and monitor patient progress, adjusting treatment plans as necessary.
- Educate patients, families, and caregivers on therapeutic exercises, mobility techniques, and injury prevention.
- Maintain accurate and timely documentation in compliance with facility policies and regulatory requirements.
- Collaborate with physicians, nursing staff, and interdisciplinary team members to optimize patient outcomes.
- Participate in care planning meetings and discharge planning activities.
Qualifications
- Active Tennessee Physical Therapist (PT) License.
- Graduate of an accredited Physical Therapy program.
- Skilled Nursing Facility (SNF) experience preferred, but not required.
- Strong clinical assessment and treatment skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively in a patient-centered, team-oriented environment.
