Job Overview:
We are seeking a dedicated and compassionate Community Liaison to join our Outpatient behavior health programs in Orange County. The successful candidate will act as liaison between the Hospitals and community based mental health professionals and agencies in a geographic area that is everchanging.
Responsibilities:
- Continuously improves and develops relationships with the referral sources, physicians, and Allied Health professionals and identifies new referral sources.
- Effectively serves as a community resource for educational and referral information and organizes program/hospital hours.
- Maintains good communication with program physicians and outsie community providers.
- Maintains a strong bond with nursing staff.
- Effectively assimilates with program staff and works in partnership with Program Director.
- Maintains availability, as needed, for patient intervention at program and in the community.
- Regularly follows up on admitted clients to ensure engagement with the program.
- Continuously develops and foster reslationships with referral sources.
- Attends to CHC units weekly.
- Effectively performs additional duties and responsibilities as position requires.
Experience:
- Minimum 1 year experience in working in-patient or outpatient psychiatric facilities.
- Strong people skills, communication and organizational abilities, and detail oriented.
- Must have means of own transportation with a current, valid CA Driver's license and auto insurance.
- Must be bilingual English-Spanish.
Job Type: Full-time
Pay: $70,304.00 - $77,334.00 per year
