Universal Registered Nurse (RN)

Are you a compassionate Registered Nurse looking for a dynamic role that combines clinic-based care with in-home nursing? We're seeking a Universal RN to provide comprehensive care for older adults across both outpatient and home settings while collaborating with an interdisciplinary healthcare team.

As a Universal RN, you'll provide skilled nursing care in both clinic and participant home settings while serving as a vital member of the interdisciplinary care team.
Clinical Care & Care Coordination
- Perform comprehensive nursing assessments and ongoing participant evaluations
- Develop, implement, and update individualized Plans of Care
- Administer medications, injections, wound care, and other skilled nursing treatments
- Implement provider orders and monitor participant progress
- Triage participant and staff phone calls, prioritizing urgent care needs
- Educate participants and caregivers on medications, disease management, safety, and healthy living
- Monitor medication compliance and provide self-administration education
- Coordinate specialist appointments, diagnostic testing, hospital admissions, and community resources
- Document care accurately and maintain complete electronic medical records
- Participate in interdisciplinary team meetings, family conferences, and care planning
Clinic Responsibilities
- Assist with participant examinations and treatments
- Prepare and maintain exam and triage rooms
- Ensure adequate medical supplies and equipment are available
- Support clinic operations and workflow
- Provide leadership support in the absence of nursing leadership
In-Home Responsibilities
- Deliver skilled nursing care in participants' homes
- Conduct home assessments and evaluate changes in condition
- Coordinate home care services and community resources
- Supervise LPNs and Home Health Aides/CNAs in accordance with regulatory guidelines
- Conduct supervisory visits and care conferences
- Collaborate with home care leadership to ensure quality outcomes
End-of-Life & Comfort Care
- Conduct end-of-life assessments and develop individualized care plans
- Educate families on the end-of-life process and available resources
- Provide emotional support and collaborate with social work for bereavement services
- Assist participants and families in understanding care options and making informed decisions
- Address spiritual care needs through appropriate community resources
What We're Looking For:
Required Qualifications
- Current, unrestricted Colorado Registered Nurse (RN) license
- Current BLS and First Aid certification through the American Heart Association or American Red Cross (required before hire)
- Minimum 1 year of experience caring for frail or elderly adults
- For home care assignments: valid driver's license, reliable personal vehicle, acceptable driving record, and current auto insurance
- Excellent assessment, communication, and care coordination skills
- Comfortable working in both clinic and home environments
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Gerontological Nursing Certification
- Two or more years of healthcare experience with an emphasis in geriatrics
- Hospice or end-of-life care experience
- Experience developing end-of-life care plans and providing family education
- Knowledge of bereavement counseling and supportive care resources
- Bilingual in Spanish, Russian, Punjabi, or Hmong
